I hereby authorize 3 Day Title LLC, its agents or assigns, to verify my past and present accounts and payoff information that are required to process my real estate closing. It is understood that a photocopy of this form will also serve as authorization.

In addition, the financial institute is granted authorization to deduct fees relating to obtaining this payoff by fax or mail from my personal account, when applicable.

The information 3 Day Title LLC, its agents or assigns, obtains is to be used in processing my closing transaction.
